The objective of the "Systems In Sync" project is to ensure that Kansas youth with special health care needs (YSHCN) and their families receive services that are:
- Culturally Competent
- Uninterrupted
- Age Appropriate
YSHCN benefit from services that meet their needs and promote healthy, productive and independent adult lives.
The grant focus is on working towards a coordinated system of care in Kansas that enhances health care access, services, and transition for Kansas YSHCN and their families. The following populations are also a focus of our project:
- Youth ages 14-25 with a special health care need
- Family and Support Team Members
- Health Care Providers
- Educators
- Transition Specialists
- Work Force Development
- Community Partners
