Call for Investment

To further enhance our efforts of collaboration, a call for investment opportunity was developed to provide funding for individuals, agencies, or organizations in Kansas to the develop or expand activities that are supportive of the grant objectives.

In year 1, we had four submissions and all four were awarded funding!

The 2010 Call for Investment Application is now available! The deadline for submissions is December 31, 2010 at 1:00 pm.


2010 Call for Investment Awardees

Kansas University Medical Center

Adults with Down Syndrome Specialty Clinic (ADSCC) @ KU Medical Center
The ADSSC will enhance their approach to health care through the addition of a Mental Health Nurse Practitioner to the team. This individual will join the Family Nurse Practitioner and the Registered Dietician to provide improved services and allow their patients the opportunity to be seen by three disciplines at one time (cutting down on the number of appointments and treatments necessary).

Kansas Academy of Family Physicians (KAFP)
KAFP will host two Patient-Centered Medical Home (PCMH) Learning Collaboratives to educate Kansas Academy of Family Physicianshealth providers about the medical home approach to health care. This will also serve as an avenue to offer assistance in transforming their practice to become a medical home and to offer support and resources for interested and engaged providers.

Kansas Enrichment Network (KEN)Kansas Enrichment Network
KEN will conduct a data assessment to identify the impact having a special health care need has on the drop-out rate. A needs assessment of after-school providers on their capacity to care for and include YSCHN in their programs will also be done. Finally, they will work with a variety of youth councils and groups to conduct a one-day leadership symposium. Throughout this symposium, youth will be given educational opportunities as well as the chance to share their knowledge and skills to enhance services received by youth and young adults.

Three Rivers, Inc
Three Rivers, Inc. Three Rivers, Inc will host a regional disability summit designed to address transition needs for YSHCN. This summit will also provide the opportunity for YSHCN and their families to share their experiences, concerns, hopes, and frustrations. This sharing will allow for the development of support groups and an increase in community action regarding pertinent issues to the community related to transition.
